Clinic Policies

Your First Appointment

Come ready to move! Please wear comfortable clothing that will not restrict your movement during your visits.

​

If you are a new patient, or have not been seen with in the past year, patient intake forms will be emailed to you upon scheduling. Please submit forms prior to your first visit.

​

If your doctor has written you a prescription, please bring a copy of it to your evaluation. No script? Thats ok! Virginia is a direct access state so you do not need a prescription for the first 30 days.

Payment

We are an out-of-network clinic, meaning we do not participate with any insurance plans.

 

We DO accept FSA/HSA accounts. Also, if your insurance company provides reimbursement for out-of-network providers, we can provide you with a superbill to submit.

Payments accepted as cash, check, credit card or Venmo

​

A 3% convenience fee will be charged for all online invoices/bill pay. Please consider paying with cash, check or ACH transfer to avoid this additional fee. 

​

Payment is always due at, or before the time of service or a $50 late fee will apply

​

All returned checks will result in a $25.00 fee

​

No refunds will be issued for any reason. Any unused package visits are valid for 1 year after purchase.

Cancellation Policy

Your appointment time is reserved just for you. A late cancellation or missed visit leaves a hole in the therapists' day that could have been filled by another patient. As such, we require 24 hours notice for any cancellations or changes to your appointment. Patients who provide less than 24 hours notice will be charged for the FULL visit amount.

Privacy Policy:

On Pointe Wellness and Rehab PLLC will maintain your privacy to the highest standards and will only disclose personal health information for the purposes of carrying out treatment, communication with other providers regarding your care, obtaining payment and for any administrative operations related to treatment or payment.

If you wish for your information to be shared with your coach, trainer, dance teacher, etc, you must complete a "Release of Medical Information" form, which can be provided at your request.

Package Policies

Package visits are available for purchase after a full Physical Therapy evaluation has been completed. 

​

Unused package visits are non-refundable. Visits are valid for 1 year after purchase. 

​

Payment is due in full at the beginning of your first package visit. No exceptions.

​

A 3% convenience fee will be added to all packages purchased via online bill pay. You can pay in cash, check or in person credit card to avoid this additional fee. 

​

Package visits are non-transferable and can only be used by the individual who has purchased it.

​

Visits can be redeemed for in-person or virtual sessions.

​

A few more things:

An electronic copy of your medical records may be requested at any time for no additional fee. There is a $10 fee if paper copies are requested.

​

Medicare patients are unable to be seen at On Pointe Wellness and Rehab for Physical Therapy. If you have Medicare, we would be happy to help you find a therapist who will accept your insurance.
